Organization of the Year Nomination Form

Thank you for your participation in the Oconomowoc Area Chamber of Commerce Annual Awards. The Chamber has been a proud host of these awards since 1985!

ORGANIZATION OF THE YEAR AWARD CRITERIA: This award recognizes a 501(c)(3) non-profit or a service club/organization making a positive impact in the Oconomowoc Area. The selection committee will review and weigh the nominated organization based on the following criteria:

Community Service/Impact
Improving Quality of Life
Serving unmet or under-served needs
Leadership
Collaboration

If you would like to nominate an organization for this award, please answer the following questions and return to the Oconomowoc Area Chamber of Commerce by noon on September 13. The organization must be a member in good standing of the Oconomowoc Area Chamber of Commerce. Self-nominations are welcome. Previous award recipients are eligible to be considered every ten years.
